When is the office closing?

The patient portal will be active until June 1, 2023 and can be accessed from this website.

How will I get my medical records?

Many of your patient records can be downloaded from the patient portal.

Otherwise, by submitting the Medical Records Release Form available on this website, you can have these records faxed to your new doctor or have them emailed to you. There will be a charge for request of more than two years worth of records.

Can I call the office?

The office telephone is closed. Please contact us by email to cbaruizmd@gmail.com or leave a message on the patient portal.

Will I need to fill out a form to get my records?

Yes, unless you are downloading them from the portal, please submit a Medical Records Release Form. Click on Forms on the home page and there will be the proper form. Please fill it out and either email it to cbaruizmd@gmail.com or send it to:

What about my insurance claims?

Although the office will close, all claims will continue to be processed. Invoices will be mailed and payments should be sent to the address on the invoice. If your have a billing question, you can email cbaruizmd@gmail.com. Please do not include confidential medical information in the email.
