Letter to my patients

Dear Friends, 

 After serving the Cary area as a doctor for the past 26 years, today I am announcing the closure of my office at 2424 SW Cary Parkway effective July 15, 2022. I do this with mixed emotions. Later this year I may begin part time work, but for the moment my main concern is your continuity of care. 

I can transfer your medical records to a doctor of your choosing or to yourself. To request your medical records please complete the Medical Records Release Form on the website www.cleobaruizmd.com.  Of course, you are always able download some of your records from the patient portal without this form. 

If you need a recommendation for your next primary care provider, please consider Generations Family Practice at (919) 852-3999. They are located off Cary Parkway at 1021 Darrington Drive, just eight minutes from my current office. There are multiple providers there and they should be able to schedule new appointments quickly. Their patient portal is the same as ours.

After July 15, our mailing address will be for anything but invoices/billing will be:

I regret this interruption in your medical care, but these last 26 years have been a very rewarding experience for me and I have enjoyed being your doctor. Please accept my best wishes for good health and happiness going forward.
